Registered Nurse (Caregiver Support)
Staff RN (Caregiver Support) Implements health promotion, prevention and management of sub-acute, chronic illness and mental illnesses Provides clinical evidence based services, monitors patient care and treatment through regularly coordinated home visits Caregiver, Veteran, and staff education of caregiver issues; facilitates psycho-education groups; continuous quality improvement activities and evaluation/consultation Skills to communicate capable of working collaboratively with all level of healthcare professionals and functions in support of caregivers, Veterans, and family members throughout the medical center Participates in direct caregiver and Veteran contacts to identify and assess caregiver stress/burden and the development and application of directed clinical treatment interventions Assist with the completion of initial, interim, Mental Health and Annual in-home assessments for active participants in the program Makes home visits to complete assessments in a timely manner and based on assessment refer veterans appropriate provider Service area varies according to the Veteran's residence, some home visits can extend as far as 4 hours away Ability to monitor complex medical issues as required Performs other duties as assigned to the position Work Schedule:
